Sinai In-Home Care is currently looking for an experienced and compassionate full-time Lead RN Care Manager.

Help seniors age in place with Sinai In-Home Care. Join an innovative team focused on personalized care, maintaining dignity, and allowing seniors to age in place safely with the support of in home caregivers. Assess clients, create individualized care plans, support and educate families. Train, delegate, and supervise caregiving staff to ensure clients have the very best support at home.

Responsibilities

Management Team Member

Assures that all services and operations are within the Oregon Revised Statutes and Oregon Administrative Rules governing in-home care agencies.

Assures all nursing services are delivered in accordance with the service plan, organization policies, procedures and protocols and in compliance with HIPAA, organization, OARs, ORSBN rules.

Assures all company, client, and employee information is handled in a confidential manner.

Advises on the in-home care policy and procedure manual by writing policies, protocols and procedures as requested by the Executive Director in accordance with overall company policies.

Serves on the Quality Assurance, Process Improvement Committee (QAPI) and actively addresses issues as they arise.

Provide relief back-up coverage in the office for other RN Care Manager or afterhours for Relief RN as needed. IE: vacation coverage or days off.

RN Care Manager

Supervises and works closely with Client Care Coordinator(s); involved with hiring, training, counselling, evaluating and terminating direct reports.

Responsible for the development and implementation of caregiver training and competency testing (initial and ongoing)

Understand best practices of home care services and implement as such. These are to include: nurse delegation services, medication services, and direct patient care services.

Supervises the delivery of care provided by direct patient care. Reviews all cases and care plans and discusses cases with Care Team members on a regular basis, at least quarterly. Assures all services are delivered in accordance with the care plan.

Trains or delegates training of all direct care personnel including RNs and Caregivers on the topics of Personal Hands-On Care, Transfers, Medication Administration, Emergency Procedures, and all aspects of safe caregiving practices.

Responsible for overall infection control, universal precautions and protocols, including maintenance of policies and procedures, implementation, staff training, records.

Client Care Management

· Responsible for overseeing Client Care from start to end of services.

· Conducts or delegates initial screenings, in-person assessments and develops care plans within required time frames, including all appropriate signatures.

· Actively communicates, through telephone calls and home visits, with the clients, HIPAA-approved client representatives, physicians and responsible parties according to company policy and state regulations.

· Triage phone-calls regarding client care during the day and working to support Relief RN after office hours and over the weekend with client-specific urgent matters as needed.

· Manage a dedicated caseload of clients receiving Homemaker, Personal and Complex care services with the support of office staff.

· Conducts or delegates follow-up visit to clients within 7-30 days after start of service date.

· Conducts or delegates supervisory visits every 90 days, or more frequently if necessary.

· Conducts 90 day Nurses Visits for clients receiving Complex Care or medication assistance or administration.

· Orient staff and caregivers to client case and care plan. Answer care-related questions by on-shift caregivers to ensure appropriate services are being delivered.

· Provide guidance and direction to caregivers to ensure accurate completion of Medication Administration Records/Narcotic Count Sheets.

· Conduct training and RN delegation to caregivers of client-specific tasks as needed.

· Inform designated office staff and Executive Director of any client issues in a timely manner, and immediately if there is any situation that may put the company, clients, or staff at risk.

What you'll need

Qualifications

· Registered Nurse in good standing with Oregon State Board of Nursing

· Strong experience working with seniors and people with disabilities preferred

Cedar Sinai Park's roots are firmly planted within the Portland Jewish Community. In 1920, the need arose for housing for the elderly, and the community responded by furnishing a 16-room Victorian home in the heart of the primarily Jewish neighborhood in Old South Portland. Originally named the Jewish Old People's Home, it served seniors at that location until the 1950s. In 1954, the Board of Trustees purchased and broke ground on seven acres of farmland in southwest Portland, where suburban neighborhoods were beginning to crop up. Today CSP continues to serve our community, residents and patients and has grown to 365 employees. Our motto is Love, Honor, Respect. We're a special team, with big hearts and high standards.
